The Supportive Living Services program provides self-directed services that help individuals live as independently as possible in their own homes. They’ll gain the skills to:
- Find a place to live and establish a comfortable home
- Make their own decisions about what care and services they want
- Build relationships and become part of their local community
CFS
Coordinated Family Services
Coordinated Family Services provide resources and support to enhance the lives of individuals residing at home with family members or caregivers. Our services help them:
- Make connections with family support services in the community
- Attain the highest possible level of independence in their daily lives
- Live and thrive with family, achieve goals, and sustain good health
