Ensure needs are met consistently over time with coordinated services and shared decision-making with individuals, families, and providers.
Connecting to Support Services
CFS coordinates and oversees the delivery of services from different providers. In addition, there are other services that many families and caregivers don’t know exist, or how they can tap into those services. We connect them to resources available in their community so the individual and caretakers have access to the opportunities and extra support they need.
CFS is a bridge to a fulfilling life, built on family, independence, and community.

Person-Centered Planning
Service plans are developed through a person-centered process. They recognize each person’s unique desires, interests, and dreams, and emphasize what’s “Important to” and “Important for” the individual’s well-being.

Customized and flexible support adapts to changing needs, simplifying service coordination. We future plan for individuals, including those living with aging caregivers, to make sure they can continue to live the life they want.
